Position Overview
PharmD Live is seeking an experienced Clinical Pharmacist to join our expanding telehealth team in Virginia. In this role, you’ll deliver pharmacist-led care coordination and medication management services that reduce risk, improve outcomes, and advance value-based care performance. You’ll support patients remotely through Chronic Care Management (CCM), Transitions of Care Management (TCM),Remote Patient Monitoring (RPM), and Medication Therapy Management (MTM), ensuring clinical quality and patient satisfaction.
