A Day in the Life
The Community Health Worker serves as a trusted frontline public health worker who partners with patients, families, primary care teams, and community organizations to improve access to care, address social drivers of health, support chronic disease self-management, and connect patients with medical, behavioral health, and community-based resources.
The CHW supports patients through health system navigation, resource coordination, outreach, education, care plan reinforcement, and culturally responsive coaching. The CHW does not provide clinical assessment, diagnosis, treatment, clinical case management, or services requiring professional licensure.
