As an Advanced Practice Provider (NP or PA) focused on Digital Care, you will be a cornerstone of this mission. You’ll provide holistic, empathetic care to patients across multiple geographies, leveraging your clinical expertise and keen attention to detail to ensure their needs are understood and addressed. Guide and oversee patients’ medical management using a holistic framework (including the 5M approach: Mind, Mobility, Medications, Multicomplexity, Matters Most).
Collaborate closely with patients’ primary care providers, specialists, and other teams such as pharmacy and specialty care to ensure continuity and comprehensiveness of care.
